Missing Information

Learn how to respond when additional information or documentation is needed to complete an ICHRA administrative matter.

Sep 11, 2026

Missing information is data or documentation needed to address an ICHRA administrative matter. A request for additional information does not necessarily indicate an error; it may mean that the matter cannot be fully reviewed using the details currently available.

Common gaps may include incomplete employee identifying information, employment status, employee class, work location, effective date, dependent details, or documentation of qualifying individual health insurance coverage. Missing or inconsistent information may delay eligibility, enrollment, allowance, coverage-verification, or other administrative matters.

When information is requested, the employer or employee should provide complete and current details by the stated deadline and using the method specified in the request. If the requested information is unavailable or the request is unclear, the recipient should not guess and should instead ask what clarification or alternative documentation may be appropriate.

Related matters may also be addressed in Employee Changes, Eligibility Updates, and Open Items.

Employers and employees remain responsible for the accuracy of the information and documentation they provide.
