An allowance change requires an effective date and a basis under the plan.
Do not make an informal individual adjustment when the underlying class, age, family information, or plan configuration is incorrect. Correct the underlying information instead.
An employee's selected premium does not determine their allowance. If the premium is higher than the available allowance, the difference remains the employee-paid portion. If the premium is lower, the unused amount is handled according to the terms of the plan.
